Welcome to 4Next 4Next is a web based application developed by Partners HealthCare System used to support the transition of patient care from the acute hospital to the post acute setting. Staff at the acute hospital can electronically assemble patient information and communicate with multiple post acute providers simultaneously and securely. This data includes pre-discharge and discharge information in a HIPAA compliant format. This clinical data is real time enabling the post acute provider to view information as it changes. 4Next provides a high quality, comprehensive discharge record that will improve the transition for the patient and the provider. Updating your facility information allows Partners Hospitals to have your most up to date information available to them when they search and initiate referrals to you. Thank you Partners HealthCare Skilled Nursing Facility Collaborative Network Application Information Partners HealthCare is accepting applications for participation in Year 6 of the Skilled Nursing Facility Collaborative Network through June 30, 2018 at 5 PM. Participation in the Collaborative Network is an opportunity for skilled nursing facilities to join a bi-directional, quality-based collaborative emphasizing quality improvement and education. Facilities who meet the minimum criteria are welcome to apply for inclusion in the 2018-2019 Network: Average DPH score of 121 (taking average of the last 3 surveys and dropping the lowest score) A CMS 5 Star rating of 3 or above To indicate interest in applying to the Collaborative Network, or for additional information, please contact the network mailbox: PHSSNFnetwork@partners.org . Facilities should receive an acknowledgement of their inquiries within 24 hours and will be emailed a link to an online application survey which they must complete by June 30, 2018 in order to be considered for participation in Year 6, which begins October 1, 2018 and will conclude September 30, 2019 (a one year contract). Important note for facilities currently members of the PHS SNF Collaborative: Facilities that are currently in the network who meet minimum criteria and are in good standing will remain in the network for 2018-2019 and do not need to complete the application and/or re-apply. Partners HealthCare Hospice Collaborative Network Application Information Partners HealthCare is accepting applications for participation in Year 1 of the Hospice Collaborative Network through June 29, 2018 at 5 PM. Participation in the Collaborative Network is an opportunity for hospice agencies to join a bi-directional, quality-based collaborative emphasizing quality improvement and education. Agencies who meet the minimum criteria are welcome to apply for inclusion in the 2018-2020 Network: Hospice agencies must be licensed to provide services in MA Hospices must be Medicare certified Hospice agency must NOT be considered "terminated or at risk for termination from Medicare." PHS Gift Policy Reminder Dear Post Acute Provider: Please note that Partners HealthCare System has a policy which prohibits employees, medical and professional staff members of Partners Hospitals from receiving gifts from any vendor of Partners. This policy prohibits all gifts from vendors to individuals who work for Partners, and gifts to Partners of items that are intended for the personal use of Partners individuals. There is no dollar threshold for the ban, gifts of any dollar value are no longer allowed. The policy explicitly includes holiday gift baskets for departments.
